  • Are You 18 Years old or older?*
  • If you are not 18 years old or older, and this is your first visit, please make sure you are accompanied by a Parent or Legal Guardian, as i will need their permission in order to perform services. 

  • For Waxing Services:

    Please make sure your hair is at least 1/4 of an inch. In order to get good results please have at least 2-4 weeks of hair growth, post shave. The longer the better.

     

    Please Do Note that Waxing services can cause redness and/or swelling and terderness.

    I have read all of the information above and confirm that my answers are true. I give permission to Maria Segura (THE WAX ROOM & CO.) to perform services we have discussed and hold her harmless from any liability that may result from this treatment. I agree to follow post treatment care which include refraining from tanning, peels, swimming, hot tubs or sauna for 48 hours after waxing. I understand that Maria Morfin (THE WAX ROOM & CO.) will take all precautions to minimize any negative reactions as much as possible.

    POLICIES
  • APPOINTMENT POLICY:

    A valid credit card will be required when booking an appointment to reserve said time.

     

    DEPOSITS:

    A $15 Non-refundable deposit is required in order to book and hold your appointment. This amount WILL go towards your service.

    Deposits can be sent Via Zelle, Apple Pay, and Venmo. Please send to (209)501-6654 or Venmo @thewaxroomandco (Maria Segura). When sending, Add name and date of appointment.

     

    SAME-DAY CANCELLATIONS/NO SHOWS:

    If for any reason you need to cancel your appointment, please do so at least 24hours before your scheduled appointment time. Any Cancellations or No Shows within the 24 hour window before your appointment will result in a 100% charge of the service you were scheduled for.

     

    RESCHEDULING:

    If you need to reschedule, please do so before the 24 hour cancellation period. You are allowed to reschedule once without forefeiting your deposit. Any rescheduling after that, will require a new deposit.

     

    LATE POLICY:

    You have a 10 minute grace period. If you arrive later than 10 minutes, i may not have enough time to perform your services without making the rest of my clients wait. Therefore, we will have to reschedule and you will be charged 100% of the services you were scheduled for.

    Please note, there is no restroom in suite. If restroom use is needed please arrive at least 15 mins early, as bathroom is located in main building on second floor.

     

    REFUND POLICY:

    NO REFUNDS OF ANY SERVICES WILL BE MADE, AS SERVICES HAVE ALREADY BEEN PROVIDED.

    NO REFUNDS OF ANY PRODUCTS PURCHASED.

    ALL SALES ARE FINAL!

     

  • BY BOOKING, YOU AGREE TO ALL POLICIES, INCLUDING: DEPOSITS, CANCELLATIONS AND NO SHOWS, RESCHEDULING, THE LATE POLICY AND THE REFUND POLICY. 

    YOU ALSO AGREE THAT BY SIGNING THIS FORM YOU ARE RESPONSIBLE FOR ALL PAYMENTS, AND THESE FORMS ALONG WITH YOUR SIGNATURE WILL BE USED IN CASE FALSE BANK DISPUTE CLAIMS ARE MADE.
