○ジストニアとは
主に中枢神経に起因し、ジストニア姿勢、ジストニア運動、動作性ジストニアを示すもの。
○口蓋ジストニアとは
下顎の開閉口や咀嚼、会話に障害を生じ、顎・舌・顔面の持続的あるいは間欠的な異常筋収縮により特徴づけられる。
しばしば頸部ジストニアや眼瞼痙攣、痙攣性発声障害を合併して頭頸部ジストニアと称される。
感覚トリックを発現することがある。

○口蓋ジストニアの症状
・顎ジストニア：食いしばりや閉口困難
・舌ジストニア：自発的な舌の捻転や突出
・顔面ジストニア：口唇突出など

○口蓋ジストニアの診断
・臨床症状に関わる問診
・筋電図
・誘発筋電図
・脳波　など

○口蓋ジストニアの治療：一般的なジストニアの治療と同様
1.薬物療法
　1-1.抗コリン薬
　1-2.ベンゾジアゼピン系
　1-3.GABA作動薬　など
2.ボツリヌス毒素療法

感覚トリックが効いても、少し薬物療法を併用したほうがいいと思っています。

○ What is dystonia
Dystonia is mainly caused by the central nervous system and is characterized by dystonic postures, dystonic movements, and action-induced dystonia.

○ What is oromandibular dystonia
Oromandibular dystonia causes difficulty in jaw opening/closing, chewing, and speaking, and is characterized by sustained or intermittent abnormal muscle contractions of the jaw, tongue, and facial muscles. It is often associated with cervical dystonia, blepharospasm, and spasmodic dysphonia, together referred to as cranial-cervical dystonia. A sensory trick may be observed in some cases.

○ Symptoms of oromandibular dystonia

Jaw dystonia: jaw clenching or difficulty closing the mouth

Lingual dystonia: spontaneous twisting or protrusion of the tongue

Facial dystonia: lip protrusion, etc.

○ Diagnosis of oromandibular dystonia

History taking regarding clinical symptoms

Electromyography (EMG)

Evoked EMG

Electroencephalography (EEG), etc.

○ Treatment of oromandibular dystonia (similar to general dystonia treatment)

Pharmacotherapy
　1-1. Anticholinergic drugs
　1-2. Benzodiazepines
　1-3. GABAergic drugs, etc.
