Rat-bite fever

Rattenbissfieber

Ratsbite fever is a rare disease occurring worldwide in Japan but particularly common in humans. The pathogens are transmitted by bites of rats, mice, squirrels and rodent-eating pet animals. It is therefore a zoonosis. After an incubation period of 1-22 days, occasionally also after two months, usually a deep dark red exanthema develops on the wound. Fieberschübe of 4-5 days change with fever-free intervals. Lymphangitis and swelling of lymph nodes, liver and spleen can be symptoms. The rats' bite fever usually heals by itself after a few weeks to months, but leads, according to some authors, untreated in 5-10% of cases to death. As a therapy, a combination of the antibiotics penicillin G with an aminoglycoside is recommended. Das Rattenbissfieber ist eine weltweit eher selten vorkommende, in Japan aber besonders häufig auftretende Infektionskrankheit des Menschen. Übertragen werden die Erreger durch Bisse von Ratten, Mäusen, Eichhörnchen und nagetierfressenden Haustieren. Es handelt sich somit um eine Zoonose. Nach einer Inkubationszeit von 1–22 Tagen, gelegentlich auch erst nach zwei Monaten, entsteht meist ein tief dunkelrotes Exanthem an der Wunde. Fieberschübe von 4–5 Tagen wechseln mit fieberfreien Intervallen. Lymphangitis und Schwellungen von Lymphknoten, Leber und Milz können Begleiterscheinungen sein. Das Rattenbissfieber heilt meist nach einigen Wochen bis Monaten von selbst ab, führt aber nach Ansicht mancher Autoren unbehandelt in 5–10 % der Fälle zum Tod. Als Therapie wird eine Kombination der Antibiotika Penicillin G mit einem Aminoglykosid empfohlen.
