I think so. I'm not that far yet in the story but there are a couple guides on steam and I read about it a few days ago. I just got to the portal in the Great Dome which the soul entity or dimensional being opened up. My understanding is that the 4th head key manifests upon your return from the other timeline.

Others have had issues with the secret weapon being completed but it worked fine for me. I'm a little disappointed that it cannot be upgraded like the rest of them.

Greed is Addiction is right, the head spawns at a specific point in the story. If you've already had an extended trip to timeline B to gather DNA from three powerful entities, then the head should have spawned at the exact place where you returned to timeline A. If you're not at that point in the story yet, then there's no head and you don't need it to progress the story.
